  • Our Free Senior Living Placement service connects you with memory care and assisted living communities that meet your loved one’s unique needs—without adding any financial burden. Having an advisor by your side is invaluable. We ask the right questions and know what to look for, ensuring your loved one receives the care they need now and in the future. We aim to help families avoid costly, disruptive transitions, providing peace of mind and confidence.

    Our services are provided at no cost to families, as we are compensated by senior living communities. However, please note that this offer applies only if you have not previously contacted any senior living community, A Place for Mom, Caring.com, or any other similar referral service. Suppose you have already initiated contact with a community or referral service. In that case, it may void our ability to work at no cost.

2. Consulting Services (Fee-Based)

Guiding Sensitive Conversations: Assistance with tough topics, like discussing when it’s time to move your loved one, handling safety concerns, or revisiting care plans as needs change.

  • Family Meetings & Home Visits: In-person or virtual sessions to assess needs, align family members, and connect you with essential resources.

  • Crisis & Future Planning: We help prepare for potential emergencies and plan for long-term care needs and costs.

3. Guidance from Afar

  • Remote: For $125, our one-hour virtual sessions provide personalized advice, covering the most common concerns dementia caregivers face, such as:

    • How do I manage behavioral changes like agitation, aggression, and wandering?

    • How do I handle repetitive questions, confusion, and memory loss in a way that reduces stress for my loved one?

    • When is the right time to consider a move to memory care or assisted living?

    • How can I cope with caregiver burnout and balance my own needs?

    • What are my area's costs and options for assisted living or home care?

  • Stress Relief Through Connection: Many people find that simply venting and getting some advice takes a lot of stress off their shoulders. Each session is tailored to give you actionable steps and emotional support, helping you feel prepared and less burdened.

4. Peace of Mind Check-Ins

  • Wellness Checks: If your loved one is at home or in a senior living community, our Peace of Mind Check-Ins provide regular wellness checks on their physical and emotional wellness.

  • Great for Families Out of the Area: Ideal for families who don’t live nearby, our service ensures that your loved one is doing well and receiving attention.

  • Family Communication: Each visit includes a detailed report, optional photos, and updates, keeping you informed and connected.

  • FaceTime Calls and Reassurance: We can facilitate FaceTime calls with your loved one or simply check in to ensure they’re comfortable and engaged.

My siblings and I first met Patti Sullivan when we were searching for an assisted living home for my mother. It was a difficult decision for us, and as the primary caregiver, I felt particularly guilty in making this move; however, Patti put my siblings and me at ease. I met Patti Sullivan in May, 2019. My wife’s Alzheimer’s symptoms were progressing and she needed more care than I could safely give her in our home. It was a very confusing and frustrating time because there many options for long-term care. Friends had suggestions, but I was not comfortable making a decision with limited information.

I made several calls to Patti to discuss my wife’s situation. Patti was always available to discuss whatever was on my mind. She is a great listener and asks the right questions. As the weeks progressed, Patti understood my wife’s Alzheimer’s presentation. This was important because my wife’s care needs are very different from other people with the same diagnosis.

I had discussions with physicians, psychiatrists and social workers, but Patti’s understanding of how Alzheimer’s affects a loved one is the best of anyone. It was as if Patti lived with us for a few weeks. Although I was not aware at the time, Patti also has a special knowledge of the caregiver(s) and our needs and desires. I decided that a Long-Term Care facility with daily activities was the best for my wife.

Patti guided me through the process and explained the different levels of care that facilities offered. Patti conducted an assessment which went very well. When it came time for my wife to start her Long-Term Care, Patti guided us through each phase My wife’s transition went extremely well. She immediately joined in all the activities and is very happy
