Patti Sullivan

Patti Sullivan founded Family Choices for Dementia after twelve years in senior living, where she witnessed families arriving overwhelmed and unprepared. They struggled with critical questions: What level of dementia care is needed? Is this community reputable? How can we avoid nursing home placement? These questions often went unasked, leading to poor decisions and additional stress.

As a Certified Dementia Practitioner, Patti created Family Choices for Dementia to bridge this gap. She now works with over 300 senior living communities and receives referral fees - making her comprehensive services completely free to families. Patti has helped hundreds of caregivers understand care options and specializes in the most difficult challenge: transitioning loved ones from home to community care, including planning for transition day itself.

In addition, Patti provides consulting services on challenging situations like taking keys away, testing services, navigating family dynamics, and helping when adult children can't get their parent to move. She offers support through Zoom meetings and personalized guidance.

A Boston College graduate with backgrounds in pharmaceutical sales and senior living, Patti serves families throughout New England from Duxbury, Massachusetts. Her mission is ensuring families have the knowledge and support needed to make informed dementia care decisions, avoiding costly mistakes and unnecessary transitions.
